Our team is ready to help you with questions about scheduling, financial policy and insurance, to make that part of the process as simple as possible. Personalized, gentle, comprehensive patient care is our number one priority.

Insurance Policy

We are happy to submit your insurance claims for you. Your estimated portion of the bill is due at the time of service; you may then receive a statement reflecting any remaining balance once the insurance claim has been processed. Please be advised that the information given by this office regarding insurance coverage for various procedures are estimates only. At your request, a pre-authorization for treatment can be submitted to your insurance carrier to establish the exact amount they will cover; this process takes 4 to 6 weeks. Details of individual insurance plans (specific coverage limits, whether yearly deductibles have been met, and maximums reached) are the responsibility of you, the patient. We request that patients take responsibility for managing disputes with their insurance carriers.

Appointment Policy

To maintain our high standards of care for all of our patients, we require you to be on time and give proper notice for any changes you require for your appointment.  If you are more than 15 minutes late for your appointment, or need to cancel or reschedule an appointment with less than 48 business hours notice, you may be subject to a $100 fee and/or half your treatment cost.

Infection Control

Infection controls and universal precautions protect clients and staff alike. Everyone benefits from rigorous infection control — you, your dentist, and the dental team. The cornerstone in a good and safe dental practice is the element of trust. You should feel free to discuss this topic with Dr. Lake and receive a straightforward answer.

Dr. Lake and our entire team follow procedures recommended by several federal agencies: the Occupational Safety & Health Administration (OSHA), the Environmental Protection Agency (EPA) and the Centers for Disease Control (CDC).

These measures include Standard Precautions and Transmission-Based Precautions:

  • Proper hand hygiene 

  • Gloves and face masks- disposed after each patient

  • Eye protection

  • Face shields

  • Gowns or lab coates

  • Chemical disinfection of countertops and all exposed surfaces after every patient with non-toxic environmentally safe CDC approved products

  • Sterilization of all equipment, non-disposable items and tools before every use

  • Disposable materials

We sterilize all reusable equipment, including dental hand pieces. We use an autoclave, a device that kills bacteria and viruses by steam, heat and pressure.

The best defense against disease is information. The more you know, the better equipped you are to make wise decisions about your health care. The more you know about our daily procedures and policies, the more comfortable you will feel.
